Health Questionnaire and Nutritional Report
Initial Questionnaire First you need to register for an Initial Questionnaire. When you have completed it and it is ready to submit (you don’t need to complete it in one sitting; your Questionnaire will be saved and available for a month), you will be asked to pay a fee of £47. You will then have access to your tailor-made nutritional report with detailed nutritional advice and plenty of nutritional information, which you can open on screen and print out if you wish. Also, if you wish, you can request contact with a qualified Nutritionist who, for a fee of £30 and with your permission, will be able to access your report and discuss it with you – although many people find that the report is so comprehensive that this is unnecessary, at least in the initial stages. You can also request contact with an Encourager, free of charge. Encouragers are not qualified Nutritionists but they have experienced Nutritional Therapy themselves and are happy to support you as you follow through with your own nutritional advice. Your report might suggest one or more laboratory tests, with details on how to arrange it. In addition to the laboratory report, for £16 you will receive an explanatory report from Nutritionhelp with appropriate nutritional advice. This on-line system is not appropriate for certain health problems (listed at the start of the Questionnaire) or for children/young people under the age of 18, but a facility is available for you to talk to one of our Associate Nutritionists about the possibility of receiving personal advice.
Review Questionnaire You are strongly advised to log in for a Review Questionnaire after three months, so that your progress can be monitored and your programme fine-tuned. Once again, a Nutritionist or an Encourager can be available to help you, if you wish. The cost of a review is just £30, representing extremely good value for money.
